The beneficiary IP or IW ,their dependent can come to ESIC Hospital Sarojini Nagar,Lucknow after following the below process-

1. IP or IW ,their dependent should first consult the Doctor at his allocated ESIS Dispensary.
2. If the treatment is unavailable in ESIS Dispensary, the concerned Doctor at the Dispensary issues a referral form to ESIC Hospital.
3. Upon arriving at ESIC Hospital, Sarojini Nagar,Lucknow with referral form, the patient can get his case registered through two modes

a Online The ESIC, Headquarters office has developed AAA App, through which appointments can be booked directly in the concerned specialty. Patients can directly go to OPD after check-in in the AAA app as per the time slot selected.
b Offline ESIC  Hospital, Sarojini Nagar,Lucknow has 2 Registration counters 1for male and 1 for female and 1 separate registration counter for Senior Citizens and defferentaly abled . Patients have to get their cases registered by the case counter clerk on the Dhanwantari module. After case registration, the patient can proceed to OPD.
4-If patient is Non- IP then he or she can get get OPD registration for OPD consultation only by paying nominal charge of Rs.10 only.
5.  The above process is for timing 9 AM to 4 PM Monday to Friday and 9 AM to 1 PM Saturday. Outside of this time, Patient can directly go to the Casualty Emergency department case counter and get treatment

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint
